How do I request a birth certificate?
Requests for death certificates can be made in person, by mail, or via fax.
You will need:
- Requesting person’s valid photo ID (or photocopy of, if requesting by mail)
Ex: driver's license or three pieces of identification (one piece of photo identification must be included)
- Name, address and telephone number of person requesting certificate
- Relationship to person named on the record (Self, parent, legal guardian, etc.)
- Signature of person requesting certificate
- Full birth name of person on the record
- Date of birth
- City of birth
- Father's full name
- Mother's full name before first married

Requests for certified copies of a marriage license can be made in person at the Clerk’s Office, through the mail, or via fax.
Requests must include:
- The names of the bride and groom at the time of the marriage
- Year of the marriage*
- Signature of the person requesting the certificate
- Payment

VETERANS’ DISCHARGE RECORDS (DD 214)
Veterans’ discharge papers (DD214) are available at the County Clerk’s Office ONLY if the veteran filed the DD214 with the Clerk following his/her discharge.
DD214s are confidential and are only available to:
- the Veteran named on record,
- the Next-of-Kin of a deceased veteran,
- Please Note: Next-of-Kin must supply proof of veteran’s death (i.e. death certificate / obituary) and proof of relationship to the veteran (i.e. birth certificates, marriage license, etc)
- Next-of-Kin include: the widow(er), child, parent, or sibling of the deceased
- Government agencies providing veterans’ benefits.
Requests for certified copies of a DD214 can be made in person, by mail or via fax. You will need:
- Full Name of Veteran
- Year of Discharge or Year Document was Recorded
- Signature of Requestor
- Photo ID of requestor (or photocopy for mail and fax requests)
- Proof of Kin-ship, if applicable (see Above)
