Passport applications are available at the Dickinson County Clerk’s Office or on-line at the State Department’s website.
The County Clerk’s Office accepts and submits passport applications to the Passport Services Office for issuance.

NEW PASSPORT APPLICANTS:
All new passport applicants are required to appear in person with their completed
DS 11 application.
Please Note: Passport applications must be signed in front of the clerk.

RENEWING PASSPORTS
You may use the renewal application (DS 82) only if you meet the following four criteria:
1. You can submit your most recent U.S. passport, and
2. You were at least 16 years old when your most recent passport was issued, and
3. You were issued your most recent U.S. passport less than 15 years ago, and
4. You use the same name as on your most recent U.S. passport OR you’ve had your name changed by marriage or court order and can submit proper documentation to reflect your name change.
Complete and sign the application. Attach to it:

PASSPORTS FOR CHILDREN:
All children regardless of age (including newborns and infants) must have their own passport and must appear in person with the parent(s)/guardian(s) to apply.
Passports for children ages 0-13 require all of the parent(s) listed on the child’s birth certificate to appear in person with their photo IDs to sign the passport application.
If both parents aren’t able to appear in person to sign the application, there are three options (Option A, B, or C below). Either:
- A. One parent must appear and submit a notarized statement of consent from the second parent authorizing passport issuance for the child. This statement can be either:
- A notarized Form DS-3053, “Statement of Consent: Issuance of a Passport to a Minor Under Age 14”
OR
- A notarized statement from the non-appearing/non-applying parent giving consent to the issuance of a passport for the minor child. This statement must include the minor’s full name, date of birth, and applicant’s relationship to the minor.
- A notarized Form DS-3053, “Statement of Consent: Issuance of a Passport to a Minor Under Age 14”
- B. One parent must appear in person and submit a notarized Statement of Consent or Special Circumstances describing in detail why the written consent of the non-applying parent cannot be obtained.
- C. One parent must appear and submit primary evidence of sole authority to apply (such as one of the following):
- Child's certified U.S. or foreign birth certificate (with translation, if necessary) listing only applying parent; or
- Consular Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing only applying parent; or
- Court order granting sole custody to the applying parent (unless child's travel is restricted by that order); or
- Adoption decree (if applying parent is sole adopting parent); or
- Court order specifically permitting applying parent's or guardian's travel with the child; or
- Judicial declaration of incompetence of non-applying parent; or
- Death certificate of non-applying parent
- Child's certified U.S. or foreign birth certificate (with translation, if necessary) listing only applying parent; or

HOW DO I CHANGE MY NAME ON MY PASSPORT?
If your name has been legally changed due to marriage or a court order within one year from the date of issuance of passport, send:
• A completed form DS-5504, U.S. Passport Re-Application Form,
• A certified copy of your marriage certificate or your name change court decree,

Photocopies and notarized copies are not acceptable for passport purposes.
NOTE: There is no fee for this service unless you require expedited service.
If your name has changed by other means and/or it has been more than one year from the date of issuance of the passport, you must submit a DS-82 renewal application and pay all applicable fees. See Renewal for information on the DS-82.

HOW LONG DO PASSPORTS TAKE?
• Passports are currently taking approximately 6-10 weeks to process.
• Expedited service takes approximately 3 weeks and is available for an additional $60 fee.
• Passport Agencies are available to assist customers with urgent travel needs (generally if you are traveling within 2 weeks) and require proof of departure.
• Most Passport Agencies are open by appointment only and require proof of your travel date or need for foreign visas.

HOW MUCH DO PASSPORTS COST?

- Age 16 and older: Two separate payments for a total of $97.
- One payment must be a check or money order in the amount of $67 payable to U.S. Department of State.
- One payment can be cash, local Dickinson County check, money order or Visa/MasterCard for $30.
- One payment must be a check or money order in the amount of $67 payable to U.S. Department of State.
- Under Age 16: Two separate payments for a total of $82
- One payment must be a check or money order in the amount of $52 payable to U.S. Department of State.
- One payment can be cash, local Dickinson County check, money order or Visa/MasterCard for $30.
- One payment must be a check or money order in the amount of $52 payable to U.S. Department of State.
Passport Renewals (Form DS-82): Application fee is $67 (check or money order payable to U.S. Department of State).

HOW LONG ARE PASSPORTS GOOD FOR?
If you were 16 or older when your passport was issued, then your passport is valid for 10 years.
If you were 15 or younger when your passport was issued, then your passport is valid for 5 years.

WHEN SHOULD I RENEW MY PASSPORT?
If possible, you should renew your passport approximately nine (9) months before it expires. Some countries require that your passport be valid at least six (6) months beyond the dates of your trip. Some airlines will not allow you to board if this requirement is not met.

MY PASSPORT WAS LOST/STOLEN. WHAT DO I DO?
There are two steps required to get a new passport.
1. First, you must report your valid passport lost or stolen immediately. This is required of anyone who loses or has a passport stolen.
2. The second step is to replace your passport.
To Report a Lost or Stolen Passport:
Complete Statement Regarding Lost or Stolen Passport, Form DS-64
Fill in as much of the passport information as you can.
• Answer all the other questions in detail.
• Sign and submit to:

In order to obtain a new passport you must appear in person at a Passport Agency or Acceptance Facility as outlined below.
IMPORTANT NOTICES
- If your still valid passport was lost or stolen, please submit the DS-64 form with the DS-11 application when you apply for a replacement at a Passport Agency or Acceptance Facility. NOTE: You must appear in person.
- The information you provide on the DS-64, Statement Regarding a Lost or Stolen Passport will be entered in our Consular Lost/Stolen Passport System.
- Passports reported lost or stolen are invalidated and can no longer be used for travel.
- If you recover the passport after you have reported it lost or stolen, please submit it to the address listed above. When you submit it, if requested - we will cancel it and return it to you. If not requested, it will be destroyed.
- Once a passport is reported lost or stolen, it cannot be re-validated.
To Replace a Lost or Stolen Valid Passport:
Complete Application for Passport, Form DS-11
You must appear in person at a Passport Agency or Acceptance Facility to submit your form.
Complete Question #18 on the application as follows:
- Write your name as it appeared on your passport.
- Write the approximate date of issue.
- Include the passport number if known.
- Under "DISPOSITION", mark the appropriate box.
- If your passport was expired, write "EXPIRED" next to the "OTHER" box.
